PRE-PROFESSIONAL Shadowing forms

The Shadowing forms are for students to get their observation hours verified at the time they are completed rather than following up with those he or she shadowed months or years after it happened. The top part of

the form should be filled out by the student and the bottom part signed by the professional. While CSU Programs (OT and PT) do not require verification of shadowing hours, there are other programs that do

and Pre-Professional Health Programs recognizes that students most often apply to multiple programs. 

NOTE: No other programs at this time require verification, but all pre-professional students should be

keeping an accurate record of those they shadow, dates, and total hours.
